In consideration of being permitted or legal guardian to participate in training at the Fastidious Jiu Jitsu facility and with their instructors, representative officers and students to use the equipment and facilities in connection with the facility, on behalf of myself, my personal representatives, my heirs, and my assigns, I hereby release, waive and forever discharge the Fastidious Jiu Jitsu Academy, and their officers and each of their respective officers, employers, students, and agents, from any and all liability of any nature whatsoever, including claims or suits at law or in equity, that I may have, for any and all personal injury, including death and property loss or damage that may result from, arise out of, or be related to my participation in the training, including my use or participates use of the facilities or equipment in connection with the training and travel to and/or from any training facility.

By signing below I acknowledge that I know, understand, appreciate the potential dangers associated with participation in the training and use the facilities and equipment in connection with the training. These hazards my include, but not limited to, minor scraps, strains, and bruises, as well as significant injuries such as broken bones, eye injury or loss, concussions, paralysis, and even death. If elected to participate in the training, It is done voluntarily and totally at participates own risk. By signing below, I further acknowledge and represent that I have read the foregoing, fully understand it, i am the legal guardian (if applicable) and sign it voluntarily as my own free act and deed. I , do hereby grant Fastidious Jiu Jitsu Academy and any assigns, licensees, and legal representatives of the Fastidious Jiu Jitsu Academy, the irrevocable right to use my name or participants name (or any fictional name), picture, portrait, or photograph in all forms and media in all manners, including composite or distorted representations, for advertising, trade, or any other lawful purposes, and I waive any right to inspect or approve the finished version(s), including written copy that may be created in connection therewith. I am of lawful age and or I am the legal guardian of the above named. I have read this release and am fully familiar with its contents.
